There are three choices for completing the Learning Assessment and Evaluation form:

For Nursing Professionals
• Complete both the Learning Assessment and Evaluation form online

• Complete the Learning Assessment online, and then print and mail the Evaluation form
  (Adobe Acrobat Reader version 5.0 or higher required)

If you successfully complete the Learning Assessment and Evaluation form, your Statement of Credit will be sent to you via US mail in 4 to 6 weeks.

For Patients/Caregivers
• Complete online Evaluation
