  A 65-year-old man was admitted to this hospital because of fever, intermittent abdominal pain, and rash. The fever began 5 weeks earlier, and palpable purpura developed 1 week before admission. A diagnosis was made.

• Outbreak of Severe Methemoglobinemia during Maritime Migration September 10, 2026
  Severe acquired methemoglobinemia was identified in 82 migrants disembarking in Sicily, 74 of whom were treated with methylene blue. Dermal absorption of N-methylaniline was identified as the likely cause.

  A 72-year-old man presented with a 2-month history of a painful lump on his neck. Cytologic analysis of fine-needle aspirate was consistent with suppurative lymphadenopathy. The lesion subsequently ulcerated and released purulent discharge.
